Decision Challenge is the set of mechanisms and rights through which a consequential decision can be questioned, escalated, paused, or overridden.
Who can genuinely question, pause, escalate or override the decision before the consequence becomes irreversible?
This is the question to bring into board papers, executive discussions, AI governance reviews, assurance work and material decisions where challenge may be unclear.

Policies, roles, workflows and controls may all exist. The organisation may appear governed on paper.
The decision gap
Everyone can raise a concern. Nobody can stop the action. Oversight exists, but intervention rights are slow, unclear, or purely advisory.
A small statement can reveal a larger infrastructure problem.
“Risk can challenge the decision.”
Can Risk pause it? Can the Board condition approval? Can the nominated human override an agent in time? Challenge without practical intervention authority may amount to observation.
